You need a yellow fever vaccine if you come from some countries in South America and West and Central Africa. Those are Angola, Benin, Burkina Faso, Cameroon, Democratic Republic of Congo, Gabon, Gambia, Ghana, Guinea, Liberia, Nigeria, Sierra Leone, Sudan, Bolivia, Brazil, Colombia, Ecuador, Peru, Guyana, and Venezuela. After the shot, you have 10 days to get to Costa Rica.

Do US citizens need a visa for Costa Rica?

US citizens do not require a visa to enter Costa Rica. They must, however, have a valid passport and a return ticket in order to leave Costa Rica within 90 days. (To return to your own nation or to travel to another country.) From the day you enter Costa Rica, your US passport must be valid for at least one day. Tourists from the United States are limited to 90 days in the country. A residence permit is required for stays of more than 90 days.
